About Us
Carolina Collaborative Community Care (4C) is an NCQA accredited 501(c)(3) non-profit organization incorporated in 2004 that offers care management and disease management services in Cumberland and surrounding counties.
4C collaborates with Prepaid Health Plans, medical providers, pharmacies, and other community based organizations to improve health outcomes and reduce healthcare costs through well-coordinated, value-based patient care. 4C partners with local agencies on contracts and grants that promote health , preschool development, and healthcare supports. 4C also provides universal newborn home visits for Family Connects NC Southeastern Region through a collaboration with the Partnership for Children of Cumberland County.
Our organization maintains a focus on population management, recognizing and addressing medical and non-medical social drivers of health and well-being to ultimately improve the quality of patients’ lives.
