RN Care Manager New!

4C is seeking an experienced RN. The nurse will work with pediatric or adult patient population to provide case management services to Carolina Access Medicaid patients. Experience working in home health, cardiovascular, hospital, ambulatory, and/or private practice settings for more than 2 years desired. Utilization review/discharge planning and previous case management experience also desired. Bachelor's degree in nursing or health related field preferred. Excellent communication and computer skills preferred. CCM Certification preferred. NC license required.

Health Check Coordinator New!

4C, a non-profit Care Management organization, is seeking a Health Check Coordinator to coordinate preventative healthcare for Medicaid children from birth to age 20. Individual will identify community resources for the designated population and identify children for health services follow up. Must possess a high school diploma, however degree in Social Work, LPN, or CNA I or II certification preferred. Strong computer skills and Microsoft Office products experience required. Two years of experience in healthcare or public service industry. Ability to work independently and manage time effectively. Must possess a current driver’s license.

Chronic Care Medical Director     

4C is seeking a Chronic Care Medical Director (MD) to work with participating physicians enrolled in the Carolina Collaborative Community Care network to develop, implement and prioritize quality improvement and care management initiatives targeting the Aged, Blind and Disabled (ABD) Medicaid population with a focus on heart failure, hypertension and/or coronary artery diseases. In addition, the Chronic Care Medical Director will work with the 4C Medical Director, Executive Director, the Chronic Care Team and other primary and specialty physicians in the community to facilitate coordination and collaboration in meeting the health and social needs of this enrolled population. 

Some position responsibilities include:

Leading Interdisciplinary Team meetings to assess medically complex enrollees and develop patient centric plans of care.  Assisting the network in improving chronic disease outcomes.  Assisting with the analysis and interpretation of the utilization management reports and other data reports generated by CCNC.  Identifying “outlier behavior” at individual practice levels and work with the practices/physicians to make the necessary improvements in the practice to achieve successful outcomes.

 

Strong organizational, leadership and planning skills required.  Excellence in verbal and written communication and experience in facilitation and group process required.  This candidate must have a strong understanding of the Medicaid population.  Graduation from an accredited School of Medicine and completion of residency with three (3) or more years of experience in providing health care services to the Medicaid population required.  This position offers a flexible work schedule to meet the needs of the 4C Network.
