Carolina Collaborative Community Care is seeking an efficient and compassionate professional to join our team as a Care Management Supervisor.
Job Overview:
The Care Management Supervisor will oversee a team of care managers and support staff who are responsible for providing high-quality care to our patients, ensuring their needs are met effectively and empathetically. This position requires a strong understanding of patient assessment, care planning, and the ability to work collaboratively with healthcare professionals and families.
Responsibilities:

Oversees efforts to ensure compliance with targeted case management processes

Reviews case management entries for proper documentation

Identifies missed high risk patients needing case management

Reviews care managers' caseloads for equal distribution

Supports quality improvement initiatives

Assesses standards of case management provided and documented per National Committee for Quality Assurance (NCQA) standards and Carolina Collaborative Community Care contractual obligations
Experience:
-
RN-Licensure in the state of North Carolina required

Care Management Certification preferred
-
Minimum of 3-5 years of clinical experience which includes familiarity with Electronic Medical/Health Records Experience managing chronic diseases

Strong computer skills, including Microsoft Word, Excel, PowerPoint, and Access Database systems

Effective written communication skills including, but not limited to, writing reports and correspondence Effective verbal communication skills, included but not limited to, speaking effectively to, providers and other staff

Strong adherence to HIPAA guidelines to ensure patient confidentiality and safety.

Lead Care Manager
Carolina Collaborative Community Care (4C) is seeking a dedicated and compassionate Lead Care Manager to join our healthcare team.
Job Overview:
The lead care manager will play a crucial role in coordinating patient care, ensuring that individuals receive the appropriate services and support throughout their healthcare journey. The individual will assist primary care providers with implementing disease management programs; participate in the development, review, and evaluation of the patient and/or family plan of care; utilize information from various sources to identify patients who would benefit from disease management and case management services; educate and engage patients in their own care management; identify social and/or medical barriers that impede health outcomes; link patients to appropriate levels of care/appropriate and supportive services; work collaboratively with a multidisciplinary inter-agency team to facilitate achievement of desired treatment outcomes, and monitor patient emergency department (ED) and hospitalization utilization rates.
Duties:

Assist in oversight of care managers to ensure high quality, standardized case management processes

Review care management entries for proper documentation and compliance with targeted case management process

Develop, assess, and coordinate holistic care management activities, with primary focus and support towards populations with mental/behavioral health needs

Enable quality, cost-effective healthcare outcomes.

Identifies at-risk populations using approved screening tool(s) and follows established care and reporting protocols

Provides cross coverage for care management supervisor when needed
Experience:

Greater than 2 years Clinical/Utilization Management/Discharge Planning/Home Care/Social Work/Ambulatory experience

Licensure in the State of North Carolina (RN)

Proficient in Microsoft Office and keyboarding

Experience in conducting patient assessments, particularly in behavioral health contexts.
Community Outreach Nurse
Carolina Collaborative Community Care (4C) is seeking a part time, (10 hours/week maximum), Community Outreach Nurse to work on a PRN basis. The Community Outreach Nurse will represent the organization at designated community-based events. (i.e. Dogwood Festival, Food/Clothing drive, etc.)
Job Description:
The individual will also work with patients per contracted programs and participating physicians contracted with 4C. The nurse will collaborate with providers to develop, implement and prioritize quality improvement and care management initiatives targeting the patient population(s) of assigned contracted program(s) and community-based programs.
The individual will also identify patients who would benefit from disease management and case management services; educate and engage patients in their own care management; and link patients to appropriate levels of care/appropriate and supportive services;
Requirements for Hire:
- Associate or Bachelor Degree in nursing with a current LPN or RN license required; CCM preferred.
- Must have a valid driver’s license and an automobile for use during work.
- Strong organizational and time management skills, and the ability to work independently are required.
In addition to being an award winning care management model, 4C offers work hours of 8 a.m. to 4:30 p.m., Monday through Friday. As a nonprofit organization, employees may be eligible for the Federal Debt Loan Forgiveness Program as well.
