WHO IS 4C?
Carolina Collaborative Community Care is a non-profit organization ( 501(c)(3) ) incorporated in 2004 and accredited by the National Committee for Quality Assurance (NCQA). We are a care management and disease management service organization that serves Cumberland County and the surrounding Counties.
