Community Connection: A 4C Blog of Events - Embracing 2026: A Bright Future at Carolina Collaborative Community Care

Welcome to 2026! Carolina Collaborative Community Care (4C) stands ready to continue its commitment to patient-centered, value-based, and community-coordinated care. Over the past year, 4C has made significant strides in improving health outcomes and enhancing the patient experience. Looking ahead, we are excited to build on this foundation and bring even more meaningful support to those we serve.
Reflecting on 2025: Progress That Inspires
Throughout 2025, the Carolina Collaborative Community Care (4C) blog shared stories of progress and innovation that highlights how we are making a difference. 4C focused on:
Young Men Mentorship Program. 4C has an ongoing partnership with Greater Life of Fayetteville (GLOF) and their “Tight’n It Up” Men’s Only Workshop. This partnership has allowed us to reach young men in the community and focus on support geared specifically towards them.
Community Events. The 4C outreach nurses and team provide services in the community at no cost including health screenings, blood pressure checks, and blood sugar checks. 4C became a staple of the Downtown Fayetteville 4th Friday events, monthly Kingdom Impact Ministries for Cape Fear’s Mobile Outreach events, and the Cumberland County Health Department’s Annual Fun in the Sun event to include other community events as needed.
Family Connects, NC Southeastern Region (FCNCSE) Newborn Support Program. 4C is the direct service provider for Partnership for Children (PFC)’s Family Connects, NC Southeastern Region Newborn Support Program. Family Connects trained RNs carefully assess newborns and mothers and discuss concrete next steps to address opportunities and concerns, including seeking immediate medical care when necessary. Family Connects trained RNs also keep the whole family in mind, recommending appropriate mental health services or medical care for other family members as needed.
Carolina Complete Health (CCH). 4C partners with Carolina Complete Health (CCH) to provide case management services for Tier I and II Medicaid clients within Cumberland, Harnett, Hoke, Moore, Robeson, and Sampson counties. Carolina Complete Health is a provider-led health plan established to deliver Medicaid Managed Care in North Carolina. Our Carolina Complete Health staff make sure that the patients under their care receive the services and support that they need.
City of Fayetteville Partnership. 4C partners with the City of Fayetteville to provide services under the Community Development Block Grant (CDBG) Reunification Services and Emergency Solutions Grant (ESG) Program. The CDBG Reunification Program provides support to qualifying individuals experiencing homelessness who are willing to transition their lives with one-way transportation for reuniting with a family member, employment opportunities, or recovery treatment facilities. The ESG grant provides services related to assisting individuals or families that are homeless or at-risk of becoming homeless by assisting with rehousing them.
Future Endeavors in the Works. Starting in this New Year, 4C continues to find ways to be able to serve our community so that the mission of patient-centered, value-based, community-coordinated care remains at the forefront. A healthy community, is a thriving community.
4C’s efforts have led to measurable improvements that can be felt in the community. Patients reported feeling more connected to their care teams and better informed about their health choices. Countless testimonies of people once hesitant to seek medical attention and care have helped change their lives for the better.
Continuing Our Mission in 2026
The 4C mission remains clear: to improve health care for our community by collaborating with patients, providers and partners for patient-centered, well-coordinated, value-based care. 4C will continue to provide services in the community, as we strive to see our patients flourish for generations to come.

Looking ahead, Carolina Collaborative Community Care (4C) will continue to focus on our core values; accountability, collaboration, excellence, and innovation. 4C will further improve health outcomes and enrich patient lives by continuing to embody the 4C vision, promoting a healthier community.
