Community Connection: A 4C Blog of Events - A Lovely Outreach of Care

Welcome to the month of February, as Carolina Collaborative Community Care (4C) continues its 2026 mission to deliver patient-centered, value-based, community care. The 4C Family Connects NC Southeastern Region (FCNCSE) team, alongside partners at Carolina Complete Health (CCH), remain committed to upholding the compassion and standard of care that define 4C’s vision of building a healthier community — not only for our patients, but for ourselves as well.

4C began the new year by continuing the Community Connection blog, reaffirming our commitment to serving Fayetteville and the surrounding communities through dedicated nursing services and expanded outreach initiatives. Throughout January, we also strengthened collaboration with our community partner, Greater Life of Fayetteville (GLOF), and our Men’s Only Workshop, “Tight’N It Up,” featuring the January theme, “New Year, New Legacy.”

In addition, 4C continued its engagement with Kingdom Impact Ministries events, where outreach nurses provided guidance and support to individuals facing limited access to essential resources. Our housing and transportation assistance programs, the Emergency Solutions Grant (ESG) and the Community Development Block Grant (CDBG), continued with the support of the City of Fayetteville. These collaborations have enabled us to broaden our reach and deepen our impact across the community in multiple ways.


4C continually seeks innovative opportunities to give back. On January 30, we participated in a successful donation and community giveaway in partnership with Circle of Brothers and Fayetteville Cares. Although February is the shortest month of the year, our commitment to advancing community health and supporting patient well-being remains unwavering. Delivering patient-centered, value-based, community care continues to be the guiding mission that 4C proudly provides to the citizens we serve.
