Community Connection: A 4C Blog of Events

We're in December, and almost to the end of 2025!



With the year winding down, we reached the month of December, 2025 and it has been an incredible year for us here at 4C. We have seen many people and patients in our community and offered what help we can; this makes us incredibly thankful that we can offer our services to the community. The holidays are ramping up in our community, but always remember that Carolina Collaborative Community Care takes the time to help our patients with many services we can offer.


We continue to work with our partners, especially those in Partnership for Children via the Family Connects of Southeastern NC:

November was also busy for us, with the Holidays make it a challenge to for new mothers (from the Cape Fear Valley, CFV, Hospital System) to get the needed baby items to make raising newborns a little easier. And our partnership with PFC have helped in this endeavor.

Then there is a massive outreach we have worked on throughout the year, such as our moments at Kingdom Impact providing our services of No-Cost Blood Pressure and Blood Sugar Screenings with our outreach nurses!

Another great outreach we do involves the tried and true, Downtown Fayetteville's Fourth Friday presence where we offered our services there too! Here's a small little video of that as well as a guest appearance from a local partner!

As we look forward to 2026, our mission remains the same as it did back in 2004, to provide patient-centered, well-coordinated, value-based care and services to our community to make it as healthy and happy as possible. With each new service, along with upholding the ones we offer; as well as partnering with others in our community, Carolina Collaborative Community Care will continue to serve and keep our community healthy for everyone, staff, patients, and kids to come!
