Can you believe it's ALREADY the month of APRIL? This year is sprinting ahead! Carolina Collaborative Community Care is right there ready to assist our patients and our partners with advocacy and outreach, along with our continued commitment to provide quality care management, and advocate where we can!

The title and ongoing series of workshops that we are partnering with Greater Life of Fayetteville.

Our partner, Greater Life of Fayetteville's "Tight'n It Up" Series is still going strong and reaching the 1 Year Anniversary this upcoming May and June.

Community Outreach

We offer our services to the community through outreach, such as blood pressure checks, provide information, or give them some health advice. Here's a video of us showcasing 4C's participation throughout the community:
